What to Bring: Insurance card (digital is okay!), identification card, and new patient paperwork. Please verify we accept your insurance here.
New Patient Paperwork: Our new patient paperwork is available for download here, or one of our staff would be more than happy to email it to you. If you do not have access to a printer, you can arrive 20 minutes early to fill it out in the office.
X-rays: Recent X-rays are required for most new patient visits. Our staff will help direct you to an outside facility, or if coming to San Juan Capistrano we would be more than happy to take them in our own office.
Previous Records: If you have previous diagnostic imaging or records pertaining to your appointment, please bring physical copies of the disk and report or verify with staff we have them electronically ahead of time. You can additionally have the records faxed to 949-940-8311.
How long you will be here: New patient visits average between 30-45 minutes. We know your time is valuable , the staff will do their best to stick to your appointment time to see the doctor.
Office location: To ensure your arrive on time, please look at the location of our offices here. We have two locations, please make sure you are aware of the location of your appointment prior to coming in.
Arrival time: Please try to be here 15 minutes prior to your scheduled appointment time. If you arrive more than 15 minutes after your scheduled appointment time, we will have to reschedule your appointment in order to be courteous to our other scheduled patients. We understand that there may be unforeseen events that cause you to be late. Please give our office a call so that we can do our best to accommodate you.
